(6) The proposed antennas would be approximately 100 feet above street grade on <br /> Colby Ave. The antennas would be mounted on the roof of the existing hospital <br /> building. <br /> (7) Not applicable--this criterion applies to tower structures. <br /> (8) The provider will be required to notify the City in the event that the facility is <br /> abandoned for a period of more than 120 days. <br /> (9) The proposal is classified as a major above-ground utility structure and is therefore <br /> not exempt from the Special Property Use permit review process. <br /> (10) This criterion allows the City to require that facilities be designed to accommodate <br /> more than one wireless service provider at a given site. Additional applications for <br /> WCFs on this site would be reviewed on a case-by-case basis. The City recently <br /> received an additional application for a WCF on this six(SPU#09-004). That <br /> application was approved and construction permits were issued by the City earlier in <br /> 2009. <br /> (I 1) Not applicable--these types of facilities normally do not require safety lights for the <br /> FAA. Navigational safety lighting already exists on the hospital in relation to the <br /> existing hclistop. <br /> (12) The City did not commission an independent third party review of this application <br /> because it found the application to be consistent with the review criteria; is located <br /> on a non-residential structure;and would have adequate screening and separation <br /> from adjacent residential uses. <br /> Conclusion: Based on the above findings,the Specific Evaluation Criteria for major <br /> above-ground utilities have been met. <br /> DECISION: The proposal is approved as submitted subject to compliance with all city, <br /> state and federal regulations. The following conditions shall apply: <br /> I. All panel antennas,shrouding and cabling shall be painted or otherwise colored to <br /> match the exterior color of the building. <br /> 2. Placement of antennas shall generally be consistent with the plans on tile at the <br /> Planning Department with this application. Minor modifications may be <br /> approved by the Planning Director. <br /> 3. City of Everett building and electrical permits arc required prior to the installation <br /> of the antennas. All work is required to meet City of Everett Building codes. <br /> 4. The Applicant shall contact the Traffic Department for a permit if any <br /> equipment/utility work is within the public right-of-way. <br /> 5. The Applicant will be required to repair any damage during construction to public <br /> infrastructure,such as curb/gutter and sidewalk. <br /> 5 <br />
