With the addition of the following conditicns, the plans are approved. 1. The conditional approval of the plans for Kama'aina Grindz was based upon the plans submitted September 10, 2U12 and the menu and HACCP submitted September 10, 2012. Any changes to the plans submitted Sep!ember 10, 2012 and the menu and HACCP submitted September 10, 2012 without preapproval from the Healtli District will void this approvai. 2. The Health District operating permit application process must be completed prior to opening for business. This facility will be classified as a medium risk food establishment with 13-50 seats. 3. Three-compartment sinks must be Natibnal Sanitatiun Foundatron (NSF) or equivalent listed with integral drainboards at both ends. 4. An indirect wasie drain (an air gap) is required for the food preparat�on sink, lhree-compartment sink, mechanical dish�vasher, ice machine, pop dispenser, and any equipment in which food or food contact equipment is piaced. 5. It unclear if the installation of a pop dispenser will be done. If a pop dispenser is installed, a reduced pressure backfiow prevention device is required at the end of the copper water pipe serving the pop dispensing system prior to the carbonation device. No copper or brass pipe/(ittings or other potentia�ly corrodible material is allowed afier the reduced pressure backflow prevention device 6. It is unclear irom the information submitted whether or not a dishwasher is proposed. If a dishwasher is proposed a three-compartment sink with integral drainboards al each end is still required. 7. Onl�� one food preparation sink is indicated on lhe floor plan. Because oi this, only vegelable preparation will be allowed at this sink. No ri�sing or thawmg under running water of raw meai, poultry, and/or seafood �.vill be ailowed without the installation of an additional and separate fcod preparation sink. If installed, the locatiun oi any additional food preparation sink must be approved prior to instailation. The sink must be N5F listed or equivaient and have at least on integral drainboard. 8. A horizontal separation of at least 16 inches or a pony wall, from the floor to al least 16 inches above the sink basin, is required between the food preparation sink (item#18) and the hand,vash sink (item #19). The oniy acceptable option to the pony wall, other than the 16•inch horizontal separation, is ihe use of a partition of at least 1 Y. inch in lhickness (similar to lhe partitions in restroom stalls) from the floor to at least 16 inches above the sink basin of a material that is walerproof. If this option is used it must also be sho�vn on the floor plan along with an accurate description of the mater�al to used and method used for installing and securing the this partition. 9.
