With the addition of the following conditicns, the plans <br /> are approved. <br /> 1. The conditional approval of the plans for Kama'aina Grindz was based upon the plans <br /> submitted September 10, 2U12 and the menu and HACCP submitted September 10, 2012. Any <br /> changes to the plans submitted Sep!ember 10, 2012 and the menu and HACCP submitted <br /> September 10, 2012 without preapproval from the Healtli District will void this approvai. <br /> 2. The Health District operating permit application process must be completed prior to opening for <br /> business. This facility will be classified as a medium risk food establishment with 13-50 <br /> seats. <br /> 3. Three-compartment sinks must be Natibnal Sanitatiun Foundatron (NSF) or equivalent listed with <br /> integral drainboards at both ends. <br /> 4. An indirect wasie drain (an air gap) is required for the food preparat�on sink, lhree-compartment <br /> sink, mechanical dish�vasher, ice machine, pop dispenser, and any equipment in which food or food <br /> contact equipment is piaced. <br /> 5. It unclear if the installation of a pop dispenser will be done. If a pop dispenser is installed, a reduced <br /> pressure backfiow prevention device is required at the end of the copper water pipe serving the pop <br /> dispensing system prior to the carbonation device. No copper or brass pipe/(ittings or other <br /> potentia�ly corrodible material is allowed afier the reduced pressure backflow prevention device <br /> 6. It is unclear irom the information submitted whether or not a dishwasher is proposed. If a <br /> dishwasher is proposed a three-compartment sink with integral drainboards al each end is still <br /> required. <br /> 7. Onl�� one food preparation sink is indicated on lhe floor plan. Because oi this, only vegelable <br /> preparation will be allowed at this sink. No ri�sing or thawmg under running water of raw meai, <br /> poultry, and/or seafood �.vill be ailowed without the installation of an additional and separate fcod <br /> preparation sink. If installed, the locatiun oi any additional food preparation sink must be approved <br /> prior to instailation. The sink must be N5F listed or equivaient and have at least on integral <br /> drainboard. <br /> 8. A horizontal separation of at least 16 inches or a pony wall, from the floor to al least 16 inches <br /> above the sink basin, is required between the food preparation sink (item#18) and the hand,vash <br /> sink (item #19). The oniy acceptable option to the pony wall, other than the 16•inch horizontal <br /> separation, is ihe use of a partition of at least 1 Y. inch in lhickness (similar to lhe partitions in <br /> restroom stalls) from the floor to at least 16 inches above the sink basin of a material that is <br /> walerproof. If this option is used it must also be sho�vn on the floor plan along with an accurate <br /> description of the mater�al to used and method used for installing and securing the this partition. <br /> 9. If due to item #8 above splashguards (equipment, partitions and/or walls) are provided on beih <br /> sides of a handwash sink, ihe distance between the spiashguards must be at least 18 inches apart <br /> (shouider width) to allow proper access to the handwash sink. <br /> 3020 Rucl:er Avenue, Suite 104 � Everett.
