Project: _ 8 <br /> Change Order no. <br /> Change Order Effective Date: <br /> Exhibit A—Description of Changed Work <br /> This change order adds two new bid items to the contract as explained below under Section A,which adds <br /> a total of$199,597.87. Additionally this change order modifies the quantities of those unit bid items where <br /> final quantities either over ran or under ran the quantities in the original bid proposal resulting in a cost <br /> reduction of$326,438.55,for a net deductive change order of$126,840.68. See Exhibit"B"for a complete <br /> list of these bid items. <br /> A. Items being added by this change order are listed below. <br /> Two new bid items are being added to the contract as a result of this change order. They are as follows: <br /> Work Order UT3076A—Sewer Replacement& Street Rehab.PCC Pavement Alternative <br /> Item#303—Force Account. Add a bid item to the Sewer Replacement&Street Rehab.PCC Pavement <br /> Alternative work order to pay for additions and modifications required of the contractor either on a time <br /> and material basis or on an agreed lump sum basis. The additional cost paid the contractor under this bid <br /> item is$119,787.11. <br /> Work Order UT3076G-Landscaping <br /> Item#300—Force Account. Add a bid item to the landscaping work order to pay for additions and <br /> modifications required of the contractor either on a time and material basis or on an agreed lump sum basis. <br /> The additional cost paid the contractor under this bid item is$79,810.76. <br /> B. Items being modified by this change order. <br /> Unit price over and under run bid item quantity modifications,(where quantities used were not as those in <br /> the original bid proposal),are being made by this change order.These are listed in the 13 pages of Exhibit <br /> "B",and result in a deduction of$326,438.55. <br /> C. Justification of the items being added by this change order <br /> Work Order UT3076A-Sewer Replacement&Street Rehab.PCC Pavement Alternative <br /> Item#303—Force Account. Additional work was required of the contractor above and beyond the work <br /> shown in the plans and specifications. A description of that work is listed below: <br /> • Place an additional 10 CY of concrete in the intersection of Hewitt&Oakes intersection. <br /> • Install cement concrete curb Type E-1 at various locations throughout the project. <br /> • Provide temporary asphalt grade transition in the Oakes Avenue intersection. <br /> • Use of asphalt grindings in-lieu-of crushed surfacing base course. <br /> • Grind,remove&haul asphalt from Hewitt/Broadway intersection. <br /> • Dig out asphalt treated base @ Lombard Avenue intersection&fill with concrete where roadway <br /> showed signs of pumping. <br /> • Provide and place a concrete roadway sub-base in the sewer trench line in Wetmore Avenue. <br /> • Usage of a 24-hour cement concrete admixture for Colby Avenue intersection,to provide for <br /> quicker traffic access. <br /> Exhibit A <br /> Page4of5 29 <br />
