CTED Interagency Agreement F03-30303-092 Page 2 3.0 REPORTS EPD shall submit quarterly progress reports on a form provided by or approved by CTED for the work performed during the previous reporting period. The schedule to submit reports is January 15, 2004, April 15, 2004, July 15, 2004 and a final report due October 11, 2004. In the quarterly progress reports, law enforcement will be required to provide data on the number of calls for assistance, cases investigated, number of arrests, orders of protection served, number of cases referred to the prosecutor by type of offense, and domestic homicides. Prosecutors will be required to include data information on number of cases pending, new cases received, charges filed, disposition of criminal cases, and evidence-based prosecution. Agencies receiving funds for direct services must report the numbers of persons served detailing the nature of victimization, number of persons seeking services who could not be served, and demographic information. EPD shall submit a year-end narrative report within thirty days after this Agreement period of performance ends. EPD shall furnish, along with the final report, one (1) copy of each curriculum, brochure, video, protocol or other materials developed with funds from this Agreement. 4.0 PERIOD OF PERFORMANCE The period of performance for this Agreement shall be effective October 1, 2003 and shall terminate September 30, 2004, unless terminated sooner as provided herein. 5.0 COMPENSATION CTED shall pay EPD an amount not to exceed $105,291.00 for the performance of all things necessary for, or incidental to, the services as set forth in Section 2.0 of this Agreement; as specified in the Statement of Work (Attachment A) and as described more fully in Section 15 of the Interagency Agreement Face Sheet. Transfer of funds between line item budget categories if the cumulative amount of these transfers exceeds or is expected to exceed ten (10) percent of the total project budget shall be subject to justification and negotiation of an amendment to this Agreement by EPD and CTED. No funds under this Agreement may be used for reimbursement of general administrative or indirect/overhead costs. EPD shall provide a county project match of$37,759.00. CONTRACTOR may expend match funds in a greater proportion to grant funds, however, CONTRACTOR must expend all match funds prior to the close of this Agreement. Expenditures of match funds must be 8 5 identified on the invoice voucher form provided by CTED.
