Project: <br /> Change Order no. I I <br /> Change Order Effective Date: <br /> Work Order 2857A <br /> Item#23—Storm Sewer Pipe,12-Inch Diameter. Seven additional linear feet of 12-inch diameter storm <br /> sewer pipe were installed above what was added by change order#1 to complete the storm drainage work. <br /> Item#25—Connection to Drainage Structure. One additional connection to drainage structure was <br /> added to the contract during construction on the project. <br /> Item#27—Drop Manhole Connection. The 13 linear feet of pipe for drop manhole connected was <br /> deleted as existing utilities in the area would not facility this type of connection to the existing manhole. <br /> Item#71—Concrete Storm Inlet. Two additional concrete storm inlets were added to the contract,(one <br /> along Commercial Avenue,the other at Alta Drive),to prevent surface water from ponding on the pathway. <br /> Item#72—Catch Basin,Type B. One type B catch basin was not installed as originally anticipated when <br /> change order#1 was written. <br /> Item#73—Asphalt Lip Curb. 732 linear feet of asphalt lip curb was not installed as earlier thought when <br /> change order#1 was written. The City of Everett is planning to overlay Commercial Avenue on the 2004 <br /> Asphalt Overlay Project. The asphalt lip curb may be installed after the overlay is completed. <br /> Item#74—12-Inch Drop Manhole Connection. One each drop manhole connection was deleted as the <br /> existing utilities in the area would not allow for a drop connection. <br /> Item#75—Type H,Catch Basin,54-Inch Diameter with Gas Trap&Floatable Material Separator. <br /> The type II,54-inch diameter,catch basin with gas trap&floatable material separator that was added to <br /> work order 2857 by change order#1 is being installed but being paid for under funds under work order <br /> 2857A. <br /> Item#76—Type H Catch Basin,48-Inch Diameter. An additional type II,48-inch diameter catch basin <br /> was added by the City of Everett during storm drainage installation work on the project. <br /> C. Additional Contract Time <br /> Fifteen(15),additional working days will be added to the contract time as a result of these amendments and <br /> additions to the contract bid proposal. <br /> Exhibit A 0 <br /> Page 10 of 10 <br />
