5 Project: <br /> Change Order no. <br /> Change Order Effective Date: <br /> Item#10—Cement Concrete Sidewalk. An additional 79.22 square yards of cement concrete sidewalk <br /> were installed do to the need to remove additional sidewalk because of under ground utility conflicts when <br /> placing signal pole bases and because of the removal of existing damaged and cracked concrete sidewalk <br /> panels. The City of Everett and Transtech Electric,Inc_have renegotiated the unit price for cement <br /> concrete sidewalk for those quantities above 125%of the original plan quantity.These are the 69.22 square <br /> yards of cement concrete sidewalk reflected in new bid item#17. <br /> Item#11 —Cement Concrete Curb and Gutter,Type A-l. An additional 11.50 linear feet of cement <br /> concrete curb&gutter,type A-1 were added to the contract do to the need to remove additional existing <br /> sidewalk because of unknown utility conflicts where the signal pole bases were being installed and <br /> replacement of existing cracked&sunken concrete curb and gutter panels close to the area of work. <br /> Item#12—Cement Concrete Curb Ramp. Two additional cement concrete curb ramps were added by <br /> the City of Everett during this project. <br /> Item#13—Incidental Asphalt Concrete,Class B. The original 7.0 tons of incidental asphalt concrete <br /> were not used construction on the project_ Only 1.50 tons were. <br /> Item#14—Existing Private Improvement Restoration. The original bid proposal had$1,000.00 set <br /> aside for this bid item. During the course of construction only$252.00 were used in this bid item,leaving <br /> $748.00 that were not spent to restore existing private property. <br /> Item#l5—Remove Beam Guardrail. Only 14 linear feet of the anticipated 20 linear feet of beam <br /> guardrail required removal during construction on the project. <br /> Item#17—Cement Concrete Sidewalk. See justification given under bid item#10 above. Those <br /> quantities above 125%of the original bid quantity are 69.22 square yards. The revised renegotiated unit <br /> price is$47.18 per square yard. <br /> Item#18—Remove Cement Concrete Sidewalk. See justification given under bid item#7 above. Those <br /> quantities above 125%of the original bid quantity are 73.62 square yards. The revised renegotiated unit <br /> price is$40.52 per square yard. <br /> D.
