8 <br /> The grievance procedures shall provide applicants and recipients with a review of the Contractor's <br /> decision before representatives of the Contractor. Applicants for and recipients of services described in <br /> the statement of work incorporated in this Agreement shall be informed of these grievance procedures <br /> and their right to seek reconsideration from the Contractor or the Division Administrator for the <br /> Department in the case of denial or termination of services and/or failure to act upon a request for <br /> services with reasonable promptness. <br /> If an applicant or recipient is dissatisfied with a response by the Contractor or Division Administrator <br /> for the Department to a complaint, the applicant or recipient may request a review by the Director of <br /> Human Services of the response. <br /> SAFEGUARDING CLIENT INFORMATION <br /> A. The use or disclosure by any person of any identifying information concerning an applicant or <br /> recipient for any purpose not directly connected with the administration of the County's or the <br /> Contractor's responsibilities with respect to services provided under this Agreement is <br /> prohibited, except as permitted by law and on informed written consent of the applicant or <br /> recipient, his or her attorney or responsible parent or guardian; Provided that the.County and the <br /> Contractor may disclose information to each other for purposes directly connected with the <br /> administration of their programs. This includes, but is not limited to: determining eligibility; <br /> providing services and/or benefits and participating in an audit; Provided further that, the <br /> Contractor shall disclose information for research, statistical, monitoring and evaluation purposes <br /> conducted by appropriate county, state, and federal agencies and their designees. <br /> B. To meet the requirement of informed consent, a written and signed release by the applicant or <br /> recipient must authorize the disclosure of information. <br /> REPORTS <br /> The Contractor shall timely provide to the County and to any state or federal funding agency such <br /> financial, program, and other reports, in such formats as required by this Agreement. <br /> RECORDS <br /> The Contractor shall: <br /> A. Retain all financial, statistical, participant, and other records (including medical and treatment <br /> records) and supporting documentation related to this Agreement for a period of six (6) years <br /> from the termination of this Agreement unless required otherwise by law. <br /> B.
