8 <br /> EXHIBIT "A" <br /> CITY OF EVERETT <br /> SPECIFIC TERMS AND CONDITIONS <br /> I. DOCUMENTS INCORPORATED BY REFERENCE <br /> In performing the services under this Contract, the City of Everett <br /> (hereinafter the Contractor) shall comply with the provisions contained in <br /> the following documents incorporated by reference and maintained on file <br /> at the Snohomish County Human Services Department, Division of Long <br /> Term Care & Aging: <br /> - Snohomish County Division of Long Term Care & Aging's Program <br /> Instructions <br /> - Contractor's grant application as submitted. <br /> II. REPORTING REQUIREMENTS <br /> The Contractor shall submit required reports on a format supplied or <br /> approved by the Division of Long Term Care & Aging. The Division of Long <br /> Term Care & Aging shall submit to the Contractor Program Instruction(s) <br /> which specify required reporting information and instructions for <br /> completing reports. Overdue reports shall delay payment to the <br /> Contractor until the next billing month. <br /> Report Titles Due <br /> Progress Report September 30, 2004. <br /> Final Report January 10, 2005. <br /> III. REIMBURSEMENT <br /> A. The Contractor must submit requests for reimbursement no later <br /> than ten (10) days following the end of each calendar month. The <br /> request for reimbursement must be submitted on forms approved by <br /> the Division of Long Term Care & Aging. The County shall process <br /> correct and properly submitted billings for reimbursement after all <br /> supporting documentation is received. Billings received after the <br /> 10th of the month will be processed for reimbursement in the <br /> succeeding month. <br /> B. The monthly billing shall be based on program expenses and be <br /> accompanied by monthly expenditure reports showing line-item <br /> expenditures corresponding to the Approved Budget. <br /> C. The County will process claims after all supporting documentation is <br /> provided in correct and proper form.
