• <br /> 8 <br /> EXHIBIT "B" <br /> CITY OF EVERETT <br /> STATEMENT OF WORK <br /> SERVICE DEFINITION <br /> The Contractor shall operate or provide for the operation of a senior <br /> center. This project provides start up or seed money for senior centers <br /> within the county currently not receiving county funding. This is one- <br /> time funding only available in 2004. <br /> H. MINIMUM SERVICE REQUIREMENTS <br /> A. With the hiring of a consultant, the Contractor will conduct a needs <br /> assessment that will investigate the specific needs of the senior <br /> community as part of developing a new Senior and Community Activity <br /> Center. <br /> B. Specific tasks for the consultant are as follows: <br /> 1. Conduct an inventory of the space used for existing facilities, <br /> programs and services. <br /> 2. Research immediate and long-term trends of Senior Centers <br /> combined with Recreational Centers. This will include: <br /> • Design and implement a community survey, <br /> • Conduct a minimum of three community meetings, <br /> • Gather and assemble information regarding programs and <br /> facilities from other agencies and providers of senior <br /> services, <br /> • Provide narratives of the information gathering process, and <br /> • Provide interim progress reports on initial findings from <br /> research. <br /> 3. Assess the future facility program's recreation and services needs <br /> based on an evaluation of information from tasks 1 & 2 (as listed <br /> above) and develop a list of alternatives for both senior services <br /> and facilities. <br /> 4. Prepare a Needs Assessment Report that documents findings from <br /> tasks 1, 2, & 3 (as listed above) which includes narrative, graphics, <br /> inserts and appendices necessary to support the conclusions and <br /> recommendations. <br /> C.
