6 SUPPLEMENTAL AGREEMENT NO. 1 <br /> To <br /> PROFESSIONAL SERVICES AGREEMENT <br /> This agreement entered into this day of August 2004, by and between the City of <br /> Everett, a municipal corporation under the laws of the State of Washington (City) and <br /> Carollo Engineers, P.C. (Contractor). <br /> Witneseth: <br /> Whereas the City and Contractor entered into an agreement on February 23, 2004, entitled <br /> "Professional Services Agreement" (Agreement) between the City of Everett and Carollo <br /> Engineers, P.C., to provide engineering services for the design of the Weyco Intertie Project. <br /> Whereas the City and Contractor desire to supplement the Agreement for the purpose of <br /> providing additional engineering services for the rehabilitation of the existing Weyco project <br /> including the following major categories of work tasks: Project Management, Meetings and <br /> QA/QC; Inspection; Rehabilitation and Bidding Assistance; and Services During Construction <br /> and Undefined Services. <br /> Now therefore, the City and Contractor do mutually agree as follows: <br /> All provisions in the Agreement shall remain in effect except as expressly modified by this <br /> supplement. The changes to the Agreement are described as follows: <br /> Amend the previous Exhibit A, Scope of Work, by adding: "Exhibit A-1 Scope of Work, <br /> Engineering Services for Inspection and Rehabilitation of the Existing Weyco Pipeline <br /> project". <br /> Amend the previous Exhibit B, Compensation, by adding "Exhibit B-1" which includes an <br /> estimated Project Cost Summary by task for the additional engineering services. <br /> Modify Paragraph 3. Time of Beginning and Completion of Performance, by deleting the <br /> existing sentence and replacing with the following sentence: <br /> "This Agreement shall commence as of the date of execution of this Agreement and <br /> shall be completed by December 31, 2007."This amounts to an extension of one <br /> calendar year. <br /> Modify Paragraph 4. Compensation, subparagraph D, which describes the maximum total <br /> compensation, by deleting the existing sentence and replacing with the following sentence: <br /> "Total compensation, including all services and expenses, shall not exceed a <br /> maximum of One hundred Five thousand Eight hundred Seven Dollars ($105,807)." <br /> This amounts to an increase of$33,907. <br /> I of 2 <br /> 21 <br />
