6 <br /> AMENDMENT NO. 12 TO PROFESSIONAL SERVICES AGREEMENT BETWEEN <br /> THE CITY OF EVERETT <br /> AND <br /> HDR ENGINEERING, INC. <br /> The City and HDR Engineering, Inc. agree to amend and modify their professional services <br /> agreement for the 41st Street Overcrossing Project Engineering Consulting Services dated <br /> June 14, 1999, (the "Agreement") as follows: <br /> 1. All terms,conditions and provisions of the Agreement, Amendments dated <br /> March 23, 2000, December 6, 2000,January 9, 2001, and August 31, 2001, Adjustment <br /> to Supplement 4 per Letter dated June 26, 2001, and Allocation of Management <br /> Reserve (letter dated June 13, 2002), Amendments dated July 17, 2002, <br /> September 11, 2002,December 11,2002, February 20, 2003, and December 23, 2003, <br /> and April 15, 2004, remain in full force and effect except as expressly modified by this <br /> Amendment. <br /> 2. Changes to the scope of work are as shown in Attachment A. <br /> 3. For this added work, payment shall be as provided in Attachments B and D, attached <br /> hereto. Project level of effort for this work, including Subconsultants,is detailed in <br /> Attachment C, attached hereto. <br /> 4. Maximum Amount Payable is increased by$463,96100 to $3,215,586.00. <br /> 5. The effective date of this Amendment is June 30, 2004.
