6 <br /> ATTACHMENT A <br /> City of Everett, 41s` Street Overcrossing <br /> Scope of Services - Construction Phase <br /> This scope of services includes construction engineering services by the Consultant for the <br /> federally funded 41st Street Overcrossing project. These services will include field survey, field <br /> observation, monitoring and testing, monitoring of and data analysis for preload, design <br /> engineering services including meeting participation, correspondence, submittal review, <br /> assistance with implementing a Construction Quality Assurance Plan to meet Washington <br /> Department of Ecology requirements, and project management and coordination as detailed <br /> below. <br /> Assumptions <br /> 1. City administrative staff will assist with clerical and administrative tasks involved in <br /> Construction Quality Assurance (CQA) and special inspection. <br /> 2. In the following text, Construction Contractor refers to the entity or individual with whom the <br /> City of Everett has entered into a Construction Agreement for the Project. <br /> 3. The City shall supply forms for daily reports, field note records and other construction <br /> documentation. <br /> 4. The level of effort for each task is limited to the amount of labor and expenses indicated in the <br /> attached budget. Additional effort beyond these limits will be considered Extra Work. <br /> 5. For the following services, labor allowances are an estimate only. The level of effort required <br /> for this work cannot be accurately predicted as it depends on issues outside of the Consultant <br /> team's control. Some of these issues include quality of Contractor submittals, number of <br /> submittals and if multiple reviews are required, and unforeseen conditions at the site. <br /> Allowance for multiple submittals due to unacceptable quality of the submittals is not included <br /> in the labor hours estimate. <br /> TASKS <br /> 1.0 NOT USED <br /> 2.0 FIELD SURVEYING <br /> Furnish field survey crew and equipment to assist the City with field surveying services: <br /> 2.1 Establish primary control points. <br /> 2.2 Field surveying of the settlement plates during preload monitoring. <br /> 3.0 ON-SITE FIELD OBSERVATION, MONITORING AND TESTING <br /> Provide on-site field observation and monitoring to observe the quality and progress of the <br /> construction to help determine if the provisions of the construction contract documents are being <br /> fulfilled, to responsibly carry out the program of inspection to guard the Consultant and City <br /> 22 <br /> Attachment A: Scope of Work Page 1 <br />
