11 <br /> • <br /> WokiKgtorcSI*Dep ti¢ <br /> Hea <br /> CONTRACT NUMBER SUBRECIPIENT* <br /> N13413 ®YES ONO <br /> * see Attachment 1,sections I and II <br /> INTERAGENCY AGREEMENT <br /> between <br /> STATE OF WASHINGTON <br /> DEPARTMENT OF HEALTH <br /> and <br /> EVERETT FIRE DEPARTMENT <br /> DBA CITY OF EVERETT <br /> THIS AGREEMENT is made and entered into by and between the DEPARTMENT OF <br /> HEALTH, hereinafter referred to as DOH, and the EVERETT FIRE DEPARTMENT, hereinafter <br /> referred to as the Contractor. <br /> IT IS THE PURPOSE OF THIS AGREEMENT to implement a fire injury prevention smoke <br /> alarm distribution, installation and education program in support of the Washington State <br /> Department of Health Fire Injury Prevention Project. <br /> THEREFORE, IT IS MUTUALLY AGREED THAT: <br /> STATEMENT OF WORK <br /> The Contractor shall furnish the necessary personnel, equipment, material and/or services and <br /> otherwise do all things necessary for or incidental to the performance of the work set forth in <br /> Exhibit A, attached hereto and incorporated herein. <br /> PERIOD OF PERFORMANCE <br /> Subject to its other provisions, the period of performance of this Agreement shall commence on the <br /> Date of Execution and be completed on September 30, 2005, unless terminated sooner as <br /> provided herein. <br /> Note: Funds provided are for the provision of services during the period beginning on the Date of <br /> Execution through September 30, 2005. <br /> PAYMENT <br /> Compensation for the work provided in accordance with this agreement has been established <br /> under the terms of RCW 39.34.130. The parties have estimated that the cost of accomplishing the <br /> work herein will not exceed $2,000 in accordance with Exhibit A, attached hereto and incorporated <br /> herein. Payment for satisfactory performance of the work shall not exceed this amount unless the <br /> parties mutually, via an amendment, agree to a higher amount prior to the commencement of any <br /> work that will cause the maximum payment to be exceeded.
