Adequate facilities must be provided for the orderly storage <br />of employee's clothing and personal belongings. <br />18. The plan review was made from informaLion you presented to our <br />office. Therefere, should there Uy any major changes and/or <br />additions to your layout plans and equipment snecifications, you <br />nust notify the Snohomish Health District of the changes. <br />21. It should be pointed out that the health department approval of <br />the finished installation (equipment and facilities) is subject to <br />th- final field inspection. Therefore, when ready, you should <br />call for the final field inspection. However, the health depart- <br />ment will make regular field checics of tne construction. <br />22. In 1980, the Snohomish Health District's Board of Health establishecl <br />a pian r>view charge for review of layout plans and specifications. <br />The fee schedule is S25.OU per hour to a maximum of $250.00. Your <br />total review time is eight (8) hours. Therefore, there wi11 be <br />a$200.00 charge. The fee must be paid prior to issuing a food- <br />service permit. <br />There are a n�mber of questions regarding your layout plans plus same <br />missing equipment specifications.
