'I'he Health District operating permit application process must be completed prior to opening for <br />business. <br />2. The food prenaration must have at least one integral drainboard. <br />3. Water heaters must be of sufficient size ta provide hot water to dishwasher and/or sculiery sinks and at the <br />same time provide hot water to all ;,�ndwash sinks. <br />-1• All foad service equipment must be listed by tha National Sanitation Foundation (NSF) for its intended <br />use. <br />�. Storage refrigerators must be listed for the srorage of potentially hazardous foods. <br />6. Extra �vall protection is required on wails behind sinks and food preparation tables. A IG inch high <br />backsplash of plastic laminate, tiber�lass reinforced plastic or equal is acceptable. Flonr to ceiling <br />protection is requimd on wall behind dishwashers. Plastic cuated hardboard is not acceptable. <br />7. All li=ht fixmres in food preparation and storage areas must be provided with covers or shatter prooF <br />bulbs� <br />8. Plumbing must meet state and local codes. <br />9. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fire <br />codes. <br />.� preoperational inspection is required prior to opening for business. At the time of inspection the <br />construction of the foad service establishment must be complete and all equiproent must be in place. <br />Incomplete construction may msult in a 565.00 reinspection fee. Please contact the Food Program oftice a <br />minimum of one week in advance to schedule an appointment. This will ensure compiiance with the Rules <br />and Reaulations of the State Board of Health for Food Service Sanitation. <br />If there are any significant changes or additions ro your layout or equipment, the Snohomish Health District <br />must be notified. <br />Please contac[ me if you have any questions.
