(Self stick vinyl or other type surfaces <br />are not acceptable) <br />13. All light fixtures in food preparation and storage areas must be provided with covers and shatterproof bulbs. <br />Hot hold ur it heat lamps must be provided with shatterproof bulbs. <br />14. Plumbing must meet state and local codes. <br />15. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fire codes <br />16. No food preparation sink, three -compartment sink, handwash sinks (except in the restrooms) or other <br />such equipment is proposed for this establishment. Because of this, no repackaging of food will be <br />allowed and no trimming, cutting, washing or processing of produce (fruits and vegetables) will be <br />allowed. This condition will remain in effect until such time as proper equipment and sinks are <br />installed and the Health District plan review, inspection, and permitting process for the proposed <br />changes have been completed. <br />A preoperational inspection is required prior to operating permit issuance and approval to open for <br />business. At the time of inspection the construction of the food service establishment must be complete and all <br />equipment must be in place and in proper operating condition. Incomplete construction or equipment operation <br />will result in a $168.00 re -inspection fee. Contact the Food Program office a minimum of one week in <br />advance of the desired inspection date to schedule an appointment for the preoperational inspection. The <br />preoperational inspection will ensure compliance with the Rules and Regulations of the State Board of Health for <br />Food Service Sanitation. <br />Changes or additions to the approved plans or equipment require preapproval from the Snohomish <br />Health District prior to implementation of the changes. <br />Any proposed changes to the approved plans for the grocery will require resubmittal of floor plans along <br />with an application for plan review and appropriate plan review fee. <br />Addition of the kitchen at a future date or any other change of food service use will require the submittal <br />of floor plans, specifications, menu, description of the food preparation process, plan review application <br />and plan review fee prior to kitchen construction, kitchen preoperational inspection, and approval to open <br />the kitchen. <br />Please contact me if you have any questions.
