We understand that pre-existing
material underneath the (4) large siab areas that were demolished was not disturbed at depth by
the slab demolition. Suficial disturbance in the existi ng capillary break material was recompacted
with a piate compactor along with some areas where the grade was raised with a combination of
518"minus crushed rock and pre-existing washed rock. The pre-existing capillary break consisted
of washed 7/8" minus rounded rock with some small concrete rubble mixed in. No density tests
were taken. Density tests do not apply to the above mix of materials. Two areas where pre-
' hed slab areas were located. These areas
re ad'acent to the demolis
existing plumbing chases we
were filled in with the stockpile of previously placed 7/6" minus washed rock mixed with some 5/S"
minus crushed rock.
Observed the placement of#4 rebar slab doweis at 24" centers in the (4) large slab areas that
were removed. Slab dowels were approximately 18" in length, em beddeu 4"or greater into the
existing slab in accordance with detail 13/S1.1. Epoxy used was Simpson SET-XP (lots
871 D4700, 11101920, 105E232E , exp. 0/2013, 2/28/2013 a nd 7/2013). Drilled holes were
cleaned, and epoxy and rebar were placed in accordance with Simpson's installation instructions
and ICC-ESR-2506. Approximately 300-400 dowels were placed.
To the best of our rnowledge, all items inspected today are in conformance with approved plans
and specifications.
Inspector: Wally Wagner
Reviewed By:
Michael J.
