6 <br /> AMENDMENT NO. 2 TO PROFESSIONAL SERVICES AGREEMENT BETWEEN <br /> THE CITY OF EVERETT <br /> AND <br /> COFFMAN ENGINEERS <br /> The City and Coffman Engineers agree to amend and modify their professional services <br /> agreement dated April 28,2003 (the"Agreement") as follows: <br /> 1. The following work is added to the Scope of Work described in Exhibit A of the <br /> Agreement: <br /> Construction Documents: <br /> 1. Coffman shall revise the Construction Documents to reflect the Owner's <br /> mark-ups. (E2.1, E2.2, E2.3, and E4.1) <br /> 2. Coffman shall provide additional information to show routing of 1 1/4" and 3/4" <br /> conduits from the first floor to the roof. (E2.1,E2.2, and E2.3) <br /> 3. Coffman shall delete the ground bar detail and add a new detail of the <br /> Quickport cable system. (E4.1) <br /> 4. Coffman shall provide a performance specification associated with the <br /> demolition of the fire-sprinkler system in the Network Room #120. (M1.1) <br /> 5. Coffman shall confirm Normal power versus Emergency power to dampers <br /> on the first floor ductwork revisions associated with Phase 2 work. <br /> 6. Coffman shall provide a new dedicated receptacle in the hallway fed from <br /> panel X1A2. (E2.1,panel schedules,load calculation) <br /> 7. Provide new stand-alone HVAC unit(Bard or similar)for Telco Room#138. <br /> HVAC unit will be circuited to power source backed-up by emergency <br /> generator. <br /> Construction Administration: <br /> 1. Coffman will respond to bidder questions, substitution requests, <br /> clarifications, and other in-house assistance during pricing. Coffman will <br /> issue an addendum if appropriate. <br /> 2. Coffman will review shop drawings and submittals (product information)for <br /> general conformance with the drawings and specifications. <br /> 3. Coffman will respond to contractor's requests for information. (RFI's) <br /> 4. Coffman will provide nine (9) site visits during the construction period. <br /> a. One (1) Pre-construction meeting, two (2) engineers in attendance. <br /> b. Six (6) Bi-monthly on-site construction meetings, two (2) engineers in <br /> attendance. (Assumed 90 day construction period) <br /> c. One (1) Initial "Punch-List" at substantial completion. <br /> d. One (1) Back-punch to verify contractor's sign-off of Punch-List. <br /> 5. Coffman will provide O&M Review during closeout. <br /> 6.
