Also, additional information is required for items <br />1-a and b, 4-a, 6, 7-a, 8, 10, 12, 13-a, 14, 15-a,b,c, <br />16, 17, and 18. <br />a. The plan review was made from information you provided <br />the Snohomish Health District. The health department <br />will confirm with you on those areas which are in <br />question. <br />b. Also, should there be any major changes and/or additions <br />to your layout plan and equipment specifications, you <br />must notify the Snohomish Health District of the <br />changes. <br />21) It should be pointed out that the health department approval <br />of the finished installation (equipment and facilities) <br />is subject to the final field inspection. When ready, <br />you should call for the final inspection. <br />22) In 1980, the Snohomish Health District's Board of Health <br />established a plan review charge for review of all layout <br />plans and specifications. The fee schedule is $26.50 per <br />hour to a maximum of $300.00 (1983 revised fee schedule). <br />Your total review time is eight (8) hours. Therefore, <br />there will be a $212.00 charge.
