Box 2097, 4522 Federal Way <br />Everett, Washington 98201 <br />DSHS # 73497-513-4 <br />Re: Luther Child Center <br />(Carpeting) <br />Dear Mr. Jacobson: <br />The specifications and drawings received August 9, 1973 have been reviewed and <br />are approved subject to submission and approval of answers to code and field <br />inspections by the State Electrical Inspector and the State Fire Marshal and <br />to conformance with all items of the reviews. <br />STATE FIRE MARSHAL REVIEW <br />The carpet- is approved provided it complies with the attached letter dated <br />April 16, 1971 where reference is made to conformance with the flammability <br />standards when tested under DOC - PP 1-70. <br />A set of stpmped approved documents is being returned for use at the job site. <br />These documents are to remain on file and available, in bound and good condition, <br />for inspection at the job site office of the general contractor at all times <br />until the project is completed and approved for licensure. It is recommended <br />that these documents be made a permanent part of the nursing home records <br />after licensure. <br />It should be noted that approval is for this work only as set forth in these <br />construction documents and any changes or alterations in/or to the plans or <br />specifications are subject to prior approval by the Washington State Health <br />Services Division. <br />Enclosed are two addressed post cards. Please fill in the date and return <br />Card ill when construction starts. Card #2 should be forwarded to this office <br />two weeks before anticpated date of completion of construction to facilitate <br />the final inspection. <br />Sincere ly <br />F.' M.
