Roberts <br />33780 Mills Rd <br />Avon, OH 44011 <br />Subject: Proposed Express Japan, 1402 SE Everett Mall Way #623, Everett <br />Dear Mr. Roberts: <br />Your plans have been reviewed wit'. the Rules and Regulations of the State Board of Health, and with the policies <br />of the Snohomish Health District. Vv' h the addition of the following conditions, the plans are approved. <br />1. The conditional approval of the plans for Express Japan was based upon the revised plans and menu <br />submitted October 1, 2013, and the HACCP submitted September 9, 2013. Any changes to these items <br />without preapproval from the Health District will void this approval. <br />2. The proposed menu includes maki rolls, which often include raw or undercooked fish and seafood. If raw or <br />undercooked fish and seafood will be used for the magi rolls, a consumer advisory is required. When raw or <br />undercooked meats, eggs, or seafoods and/or unpasteurized juices are offered for service or for sale as <br />ready to eat, then a consumer advisory must be available. The raw or undercooked food items must be <br />clearly identified as such on the menu or on a sign clearly visible to the patrons and must also indicate the <br />increased risk of food borne illness associated with the consumption of raw or undercooked food items. <br />Additional consumer advisory details are included on the enclosed information sheet. A copy of the actual <br />customer menu that will be used that includes the consumer advisory must be submitted prior to the <br />-- — requestfora preoperational -inspection, and -must also indicate whichitemson the menu will be. -- - <br />served raw or undercooked. Approval to open will not be given without submittal and approval of the <br />actual customer menu and consumer advisory. <br />3. The proposed menu indicates that maki rolls will be served. Maki rolls often include raw or undercooked fish <br />and seafood. If raw and or undercooked fish and seafood will be served proper parasite destruction is <br />required for raw fresh or saltwater finfish, crustaceans and other forms of aquatic life (including alligator, frog, <br />aquatic turtle, jellyfish, sea cucumber, and sea urchin and the roe of such animals) (excluding birds or <br />mammals), and all mollusks. The parasite destruction procedures must follow the requirements as noted in <br />the Washington State Retail Food Code, WAC 246-215 Part 3 Subpart B 03425. The specific and complete <br />parasite destruction process, for each raw product, meeting the requirements of WAC 246-215 Part 3 Subpart <br />B 0'3425 (or documentation of the compliance with the destruction process for each raw product from the <br />supplier/suppliers) must be submitted prior to the request for a preoperational inspection. Approval to open <br />will not be given without submittal and approval of the parasite destruction documentation. <br />4. An indirect waste drain (an air gap) is required for the food preparation sink, three -compartment sink, <br />mechanical dishwasher, ice machine, pop dispenser, steam table, walk-in refrigeration, and any equipment in <br />which food or food contact equipment is placed. Please note that a direct drain to a grease trap is not <br />considered an indirect drain even if the grease trap is indirectly drained. <br />5. A reduced pressure backflow prevention device is required at the end of the copper water pipe serving the <br />pop dispensing system prior to the carbonation device. No copper or brass pipelfiittings or other potentially <br />corrodible material is allowed after the reduced pressure backflow prevention device. <br />6. The equipment list submitted indicates that the food preparation sink has no drainboard. The Floor plans <br />indicate the food preparation sink to have a drainboard. Each food preparation sink must have least one <br />integral drainboard <br />7. It cannot be determined from the floor plan or information submitted whether or not a chemical dispensing <br />system will be installed at the mop sink. Please note that the use of screw -on type 'wye' adaptors at the mop <br />sink faucet for use with chemical dispensing systems is prohibited. Additionally chemical dispensers cannot <br />be connected to the mop sink faucet. The chemical dispenser must have a separate water connection.
