Because of this on ly vegetablelfruit <br />preparation will be allowed at this sink. No rinsing or thawing under running water of raw meat, poultry, and or <br />seafood will be allowed without the installation of an additional and separate food preparation sink. If installed, <br />the location of any additional food preparation sink must be approved prior to installation. <br />This facility has only one under counter refrigerator. No advanced preparation of foods that require <br />cooling and no cooling of leftover "oods will be allowed unless a walk-m refrigerator or blast chiller <br />(rapid temperature pull down refrigerator) is installed or other Health District approved cooling <br />method is in place. Approval of cooling methods other than in a walk-in refrigerator must be approved <br />prior to final plan approval. A revised HACCP is required for any proposed cooling method. Please <br />note that if any approval for cooling of food is given the risk classificat�on for the establishment will <br />change to a high risk food establishment and an additional permit fee for the ciassification change <br />will be charged. <br />If during routine inspections it is found that the current amount of refrigeretion in inadequate for the <br />refrigerated storage needs of the facility, then the number of inenu items must be reduced to a level <br />acceptable for the facility's amount of refngeration. The menu item reduction will remain in effect until <br />such time as additional refrigeration is installed and approved by the Health District. <br />Due to the refrigeration limitation of this facility, no increase in menu items will be allowed without the <br />installation of additional refrigeration and preapproval from the Health District. The amount of and <br />location of the additional refrineration must be approved prior to instailation. <br />The conditional approval for less than the minimum required 144 cubic feet of refrigeration is based <br />upon the information submitted during the plan review process (including that all milk used for the <br />tea is in powder form), is for the current facility owner only, and is not transferable with ownership <br />changes. <br />A preoperationai inspection is required prior to operating permit issuance and approval to open for <br />business. At the time of inspection the construction of the food service establishment must be complete and all <br />equipment must be in place and in proper operating condition. Incomplete construction or equipment operation <br />will result in a 51�0.00 reinspection fee. Requests for a preoperational inspection must be received at least <br />one week in advance of the date being requested for thoi tment �for t ent reot erat onalP nspection � The <br />minimum of one week in advance to scheduie an app P P <br />preoperational inspection will ensure compliance with the Rules and Re ulations of the State Board of Health for <br />Food Service Sanitation. <br />Changes or additions to the approved plans or equipment require pre-approval from the Snohomish Health <br />District prior to implementation of the changes. <br />Please contact me if you have any questions.
