D. Levels of Accomplishment- Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Legal Assistance (HJP) 30 individuals assisted <br /> to low income residents 3 persons/households during 10 month grant <br /> of the City of Everett assisted per month term <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 300 persons/households to be served <br /> by the HJP (Housing Justice Project) 30 persons/individuals to be served <br /> countywide, 1/3 are residents of during 10 month grant term <br /> Everett. <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Staffing <br /> [Add Provide a list of staff and time commitments to be allocated to the activity specified in I.A. above] <br /> The Housing Justice Project coordinator is Todd Hubbard, an experienced attorney with a passion <br /> for seeking justice for low income persons. Volunteer attorneys are supervised and/or mentored by <br /> the Mr. Hubbard or other senior volunteer attorneys until the coordinator is satisfied that the attorney <br /> has the requisite expertise to provide high quality legal advice and representation to the client and <br /> understands the related non-legal needs of the client and is able to make appropriate referrals. <br /> Mr. Hubbard is currently employed as a full-time staff attorney (38+ hours per week). With the level <br /> of funding available through this grant, we can sustain these much needed weekly hours, including <br /> presentations to the community. <br /> II. Time of Performance <br /> Services of the Subgrantee shall start on the 1st day of August, 2016 and end on the 31st day of <br /> May, 2017. The term of this Agreement and the provisions herein shall be extended to cover any <br /> additional time period during which the Subgrantee remains in control of CDBG funds or other <br /> A-3 <br />
