Scope of Work: involves, but is not limited to all labor, materiais and equipr.�ent for <br />sawcutling, breaking, removing, disposing of concrete debris and wet Gase marerial, <br />replacing base malerial, compacting, formmg, doweling, placing a pre-cast drainage <br />box and oil separator C.B., setting re-bar, pouring, finishing an;7 seaLng the new <br />concrete threshhold and ciean up of construcnon sue. <br />2. Contract term shall be thirry(30) calendar days for acwal construction of the threshhold in <br />two ph:+ses and waiting 28 days aRer the phase 2 threshhold is complete to cure be(ore <br />firetruck use. <br />4. Conslruction phasing: <br />Phase 1: Replace Ihe westedy threshhold doing i[ in one pour and wailing 7 curing <br />days as noted on Drawing no. I. of � be(ore any vehicle use. Usc high early <br />slrength add mixture. <br />Phase 2: As soon as the first phase is poured, replace the easterly bay threshhold <br />doing the pour in one pour and waiting the full 28 calendar days (or the <br />concrete to cure be(ore any vehicle use. 'Che easterly bay is used by a 60,000 <br />Ib. laddcrtruck. <br />5. The Contractor shall obtain and keep in force during lhe term of tne contract, <br />�ublic liabiliry and property damage insurance w�th companies or through <br />sources app:oved by the S4�te Insurance Commissianer pursuant .o Title 48 <br />RCW. Minimum amoun�s of covenge shall be $500,000 (or death or injury <br />lo any one person, 51,000,000 for death or injury to two or more persons in <br />any one oce�rrenec and 5500,000 (or property damage in any one occurrence <br />with any aggrcgate property damage covenge of 5500,000 for Iwo or more � <br />occurzences. City shalt be an addinonal nvned insured on said insurance in <br />a form acceptable to the City Atlomey. Provide Owner with copy of <br />insurance certificate prior to beginning any work. <br />6.
