� <br />Cre�tive �ngineering Options �Nc. <br />A firm practicing in the geosciences <br />February 23, 2d02 <br />Mr. Shawn & Ms. Kathy Hoban <br />500 Laurel Street <br />Everett, Washington 98201 <br />Subject: Final Monitoring Re�ort <br />Foundation Construction <br />Residential Addition <br />500 Laurel Street <br />Everett, Washington <br />[City Plan Check No. CC107010] <br />Dear Mr. & Ms. Hoban: <br />00-2267-1 <br />As requested, we have provided geotechnical construction monitoring services <br />related to the installation of a series of foundation supporting driven-in-place steel <br />pin-piles around the perimeter of the residential addition at the subject site. The <br />primary purpase of our services were to verify that the pin-piles were driven in <br />accordance with the original project specifications and to the appro�riate refusal <br />criterion, and to verify their allowable axial load capacity. For informational <br />purposes we have provided copies of our Field Construction Inspection Reports as <br />an attachment to this letter. <br />Your subcontractor, McDoweil File f<ing, Inc., installed a total of thirty-five (35) pin- <br />piles; thirty-three two inch [nominalJ diameter and two [piles 31 and 32] four inch <br />diameter piles. The two inch piles consisted of segments of Schedule 80 "strong" <br />black iron pipe and the four inch pipes were of Schedule 40 material. Where <br />appropriate, the segments of pile were connected with a"force-fiY' connector. These <br />pin-piles were driven to bearing depths of between approximaxely 7' - 6" and 16' - 0" <br />below the foundation subgrade elevation. The pile� were driven either N�ith an air <br />compressor operated ninety (90) pound jack hammer or a small trackhoe mounted six <br />hundred and fifty (650) pound hydraulic hammer. <br />After installation our representative monitored the proof testing of virtualiy all of the <br />installed pin-piles.
