The conditional plan approval for Amante Piva 8 Pasta is based upon the plans, menu, • <br /> HACCP and other information submitted during the plan review process. Any changes to <br /> the plans, menu, HACCP and other information submitted without preapproval frcm the <br /> Health District will void this approval. <br /> 2. The HeaRh District operating permit application process must be completed prior tu <br /> opening for business. This faciiity will be classified medium risk food establishment with <br /> seats. Permit applications submitted on or after December 1, 2011 will be subject to the <br /> permit fees in effect at the time of the application submittal. <br /> 3. The three-compartment sinks must have integral drainboards at both ends. <br /> 4. An indirect waste drain is required for the food preparation sinks, three-compartment sink, <br /> mechanical dishwasher, ice machine, walk-in refngeration and neezers, and any equipment <br /> in which food or food contact equipment is placed. Please note that a sink drained directly <br /> to a grease trap does not meet the requirement for an indirect drain. Sinks requiring indirect <br /> drains are required to have an air gap prior to the grease trap. <br /> 5. A horizontal separation of at least 16 inches or a ve�tical paRition 16 inches in height is <br /> required between the handwash sinks and all other sinks and food preparation areas. <br /> 6. �Nater heaters must be of sufficient size to provide hot water to dishwasher and/or scullery <br /> sinks and at the same time provide hot water to all handwash sinks. <br /> 7. Hot water must be available to all handwash sinks within 15 seconds. The hot water for the <br /> facility must not be used, at any plumbing fixture, for a minimum of 6 hours prior to the <br /> preoperational inspection. This is required so that proper testing of the 15-second time <br /> requirement for hot water availability can be completed. <br /> 8. Based upon the plumbing information submitted, it is unclear whether or not chemical <br /> dispenser systems for cieaning and sanitizing chemicals will be provided at the mop <br /> Qanitor) sink. Please note that the use of screw-on type 'wye' adaptors at mop sink faucets <br /> for use with chemical dispensing systems is prohibited. Chemical dispensers cannot be <br /> connected to the mop sink faucet.
