o I"
To: Brent and Sherry Wiggins, Chuckwagon Accessories, 2804 Grand
Street, Everett, Washington 98201
August 25, 1983
Page 2
All equipment must be National Sanitation Foundation (N.S.F.)
or equal, and installation is to be done to facilitate cleaning.
All multi -use food -contact surfaces shall be properly washed,
rinsed, and sanitized at least daily in the utility sink.
Any significant deviation from this plan review must have prior
approval.
Presently the only restrooms on this floor are shared by another
food establishment, the public, and now will be by this concession.
This is not acceptable. It is understood and agreed, and a
condition of this plan approval for Chuckwagon Accessories that
within four months three new restrooms shall be established on
this floor of the building which will provide separate facilities
especially for each of the food operations• on this floor.
A 1984 operating permit is not to be issued before these restrooms
are operative.
It appears from the standpoint of both convenience of work and
plumbing, that arranging equipment starting in the southwest
corner the chest freezer, the dipper well -utility sink, and then
the handsink all on the west wall.
Before an operating permit can be issued E final inspection must
reveal no food code violations. When reacy for that inspection
do call.
There is a plan review charge of $26.50 per hour. Your charge
is for two and a half hours, $66.25. This fee must be paid
within thirty days and of course before ar, operating permit can
be issued.
Very truly yours,
Howard Wood, R.S.
Environmental Health Specialist
HW:sei
CC: City of Everett Building Department
