Rev. 10/2/72 <br />ADDENDi1M TO FHA COMMITMENT <br />SI FET OF' <br />DATE <br />HUD -`HA SEF.TTLE AREA C FICP <br />FH6 �;E NO. <br />ADi _.oS <br />Specific commitment conditions (applicable wben checked) <br />Note: All required repairs must be completed in a professional manner. <br />VC11 0 If the mortgage is to be insured under Section 235(i) the seller, as defined by the <br />Secretary, will execute an agreement, in the form prescribed b-r the Secretary, to <br />reimburse HUD for expenses incurred in repairing defects found eligible for repair <br />under Section 518(b). <br />VC12 P Submit certification from the local authority having jurisdiction that the subject <br />property is in compliance with the Local Minimum Kousing Code. <br />VC13 f`-' Submit certification from a state licensed pest control operator that wood destroying <br />organisms, fungus and/or rot damage in all of the dwelling, interior and exterior, have <br />been eliminated. A "Standard Notice of Work Completed" or a report form indicating no <br />infestation may be submitted as certification. Note: All repairs must be completed <br />in conformance with local professional building standards and local building codes. <br />VC14 G% f/ (a) This commitment is subject to receipt of clear certificate(s) for (electrical, <br />plumbing, roofing, and heating). The required certificate(s) is (are) being ordered <br />and will be paid for by HUD. Cost of additional certificates or required repairs will <br />be borne by the Seller." <br />Q (b) Reinspection to examine the following system(s) not in operation at the tine <br />of our appraisal is required: <br />Q plumbing Q heating f] electrical <br />Correction of any deficiencies found is a specific condition of this commitment. <br />Indicated system(s) shall be operable and inspection completed prior to the -equest <br />for conversion to a firm commitment. <br />VC15 1�7 Furnish evidence from the local health authority indicating approval of the <br />Q individual water supply system; Q individual sewage disposal system. <br />VC16 Q Submit certification from the state electrical inspector that the service panel and <br />electrical distribution system are adequate, in good condition, and meet state electrical <br />requirements applicable to subject dwelling. <br />VC17 0 (a) Q re -roof O repair roof of Q dwelling and/or Q garage with minimum <br />235# self sealing composition shingles or #1 cede.: shingles, and repair sheathing and <br />install new flashings as necessary. Remove all old roofing when two or more Dyers <br />exist. <br />Z] (b) Repair built-up roof: Remove all gravel and counter flashing frwm roof', clean <br />ourface to accept hot mop. Provide minimum 15 lb. felt, hot mopped and flood coated. <br />Replace gravel and flashing and properly seal all gutters and apertures. <br />O (c) Re -roof: After removing all old roofing, install min. 3-15# felts and 400# <br />gravel. <br />VC18 0 Paint all exterior metal and wood trim of 0 house and/or Q garage after adequately <br />preparing surface. <br />VC19 LlPaint entire exterior of Q house and/or d garage including trim. Prepare surface <br />by repairing damaged areas, remove all loose paint and blisters and apply an undercoat <br />to all bare wood. <br />VC20 Q Remove the existing floor covering in the following rooms: <br />Replace with new resilient floor covering over :;udtable lmderlayment after making <br />necessary repairs to subfloor. Carpeting not acceptable in kitchen and bath areas. <br />VC21 z] Repair, paint and caulk all window sash and doors to operative condition. Minimum of <br />one window shall open in each roam. <br />VC22 0 Install a 3/4" non-adjustable temperature and pressure relief valve on hot water tank; <br />and a 3/4" dis^harge line to outside or to an interior drain. Rating of valve ❑hall <br />not be greater than pressure rating of water tank. <br />
