PLEASE ATTACH THE FOLLOWING INFORMATION: <br />1. Vicinity map showing the location of the property in relationship to <br />major streets. <br />2. A site plan (drawn to a scale of not less than 1" - 10' or more than 1" <br />100 % depending upon the size of the property, containing the <br />following: <br />All dimensions of property and number of acres or square <br />feet in site. <br />Location, dimensions and square footage of existing and <br />proposed structures in relation to the site area boundaries. <br />Parking and loading areas. <br />Traffic access and circulation. <br />Open space, undisturbed areas, landscaping, refuse and <br />service areas, utilities, signs and yard setbacks. <br />3. List of names and addresses of property owners (not the address of a <br />mortgage lending company) within 300 feet of the subject property, to <br />be typed on Address List Form. <br />Property owners' names may be obtained from the County Assessor's <br />Office, Third Floor, County Administration Building. <br />In the case where a mortgage company is listed as owner in the <br />assessor's records, the purchaser's address may be found by <br />looking up their name in the telephone directory or the City <br />directory (Polk's). If their name cannot be found in either if <br />those directories, the mortgage company name and address may be <br />used; however, in that case, it should be used as written in the <br />assessor's files including the purchaser's name and account <br />number. <br />4. Narrative Statement: A detailed description of the variance being <br />requested and a paragraph addressing each of the following specific <br />points which are the facts and conditions that must exist in order for <br />the Board to grant a variance: <br />A. That there are exceptional or extraordinary circumstances or <br />conditions applying to the subject property or as to the intended <br />use thereof, that do not apply generally to other properties in <br />the same vicinity or zone; <br />B. That such variance is necenuary for the preservation and <br />enjoyment of a substantial property right of the appellant <br />possessed by the owners of other properties in the name vicinity <br />or zone; <br />
