• <br /> 9 <br /> Project: Water Filtration Facility Facility Improvement Project <br /> • <br /> • <br /> Change Order no.3 <br /> Change Order Effective Date: January 30,2006 <br /> Exhibit A—Description of Changed Work <br /> Modify the dollar amount under existing bid item#4–"Additions and Modifications to <br /> the Contract",with a$10,000.00 increase for work change directive#45. <br /> Work Change Directive #45. Revise the Alum/Polymer Storage Building insulation <br /> from 5-inches of International Cellulose Inc. K-13 to 4-inches of Icynene and 1-1/4 <br /> inches of International Cellulose Inc. Ure-K. Lump sum price to include all costs <br /> complete, including scaffolding. <br /> JUSTIFICATION: <br /> Contract plans called for the ceiling and walls of the Alum/Polymer Storage Building <br /> to be insulated with K-13 Spray-On-System as manufactured by International <br /> Cellulose Corporation or acceptable substitute. Insulation was to be applied in <br /> accordance with the manufactures recommendations. The insulation subcontractor <br /> proposed a rigid insulation board as a substitute for the Spray-On-System. Water <br /> Filtration Plant has experienced rat infestation with rigid board insulation, thus the <br /> proposed substitute was rejected and the contractor directed to proceed with the K-13 <br /> Spray-On-System. K-13 Spray-On-System was applied to the ceiling and walls.The <br /> K-13 product within a short period of time failed to adhere to the ceiling and fell to • <br /> the floor of the building. After significant research and time the City of Everett chose <br /> to have installed 4-inches of Icynene foam insulation which required topping with 1- <br /> 1/4 inches of International Cellulose Inc. Ure-K for fireproofing. <br /> Hisey Construction forwarded a$16,681.67 additional cost request, on behalf of its <br /> insulation subcontractor, for installation of this revised foam insulation and fire <br /> proofing system, for consideration by the City of Everett. The insulation <br /> subcontractor's position was that they#1 requested an alternative insulation method <br /> be used,that was rejected and, #2 the ceiling they had sprayed the K-13 Spray-On- <br /> System to was much to flexible and moved,(expanded and contracted) with <br /> temperature fluctuations causing the failure.Note: Work change directive #11 <br /> changed the exposed roof fastener system to a concealed fastener system at the <br /> request of plant staff,as existing buildings,which have the exposed fastener system, <br /> leak. <br /> Construction management staff,Hisey Construction, and its insulation subcontractor <br /> met to negotiate costs for this added insulation work and settled on$10,000.00.
