6 <br /> Proposal Form A—Proposer Qualifications,Representations and Warranties (page 2 of 4) <br /> Proposer Name: <br /> A.5 Proposer's Release Facility and Storage Lot(s)—Prior to contract performance, Proposer shall comply <br /> with all requirements of applicable zoning code(s)for its release facility, primary storage lot, and <br /> secondary storage lot. As Attachment 2 to this Proposal Form A, Proposer shall provide the following <br /> information: <br /> 1) Street address of the release facility. <br /> 2) Square footage of primary storage lot available for storage of impounded vehicles. <br /> 3) A sketch of the release facility and primary storage lot including permanent or temporary <br /> buildings on the lot, and adjacent streets or alleys. <br /> 4) A copy of the most recent land use permit(s)for the release facility and primary storage lot <br /> 5) Evidence of whether the release facility and primary storage lot is owned, leased or held as an <br /> option to purchase or lease by Proposer. Any option to purchase or lease property as a storage <br /> lot must be firm and in writing. Proposer shall submit a copy of such option. <br /> 6) Street address of secondary storage lot. <br /> 7) Square footage of the secondary storage lot available for storage of impounded vehicles. <br /> 8) A sketch of the secondary storage lot including permanent or temporary buildings on the lot, <br /> and adjacent streets or alleys. <br /> 9) A copy of the most recent land use permit(s)for the secondary storage lot <br /> 10) Evidence of whether the secondary storage lot is owned, leased or held as an option to purchase <br /> or lease by Proposer. Any option to purchase or lease property as a storage lot must be firm <br /> and in writing. Proposer shall submit a copy of such option. <br /> A.6 Proposer's Tow Truck and Tow Truck Equipment-Proposer submits as Attachment 3 to this <br /> Proposal Form A, a list of all towing equipment owned or leased by the Proposer or held as an option for <br /> purchase or lease by the Proposer. The list shall include: <br /> Class of Tow Truck <br /> Make <br /> Model <br /> Year of manufacturer <br /> Vehicle License Plate Number, if available <br /> Indication whether the equipment is owned, leased or to be purchased or leased by Proposer. <br /> A.7 Communication Capabilities—Proposer has a published telephone number. Prior to contract <br /> performance, Proposer shall have a dedicated facsimile number and equipment, shall be capable of <br /> receiving and transmitting communications by email,shall be capable of providing reports using <br /> Microsoft Excel or ASCII format, and shall provide a compatible ring down circuit between its dispatch <br /> office and EPD Communications. <br /> 30 <br />
