• -Proposal Firm-A—Proposer Qualifications,Representations-and W-arranties-(page3 of-4) --- Proposer Name: A.8 Subcontractors—Proposer submits as Attachment 4 to this Proposal Form A, a list of all subcontractors it will use in performing the requirements of the agreement resulting from this RFP. A subcontractor is any separate legal entity used to perform requirements of the proposed agreement. The list shall include the firm's name, contact person and title, mailing address,telephone number, fax number and a detailed description of the service(s)to be subcontracted. Proposer shall also attach a copy of the letter from the subcontractor stating its commitment to perform the services(s)so described. A.9 References—The City will enter into an agreement only with a Proposer(s)having a reputation of satisfactory performance. The Proposer's ability to provide timely service; knowledgeable, conscientious, and courteous towtruck drivers and office staff; reasonable care and skill; invoicing consistent with contract pricing, etc., are important to the City. The list shall include the agency's name, contact person and title, mailing address, telephone number, and a detailed description of the service(s)provided. The list shall also indicate whether the contract was terminated and the reason for such termination. 31
