Ren Callahan, the City of Everett Building <br />Code does not permit you to set aside one of the building complex rest- <br />rooms for your food-service establishment. Consequently, an employee <br />restroom will be required by the health department. AC the time of the <br />field inspection of your equipment and facilities, it caas mentioned that <br />the Snohomish Health DistricC caould issue �ou a provisional permit for <br />thirty (30) days so you could malce the necessary arrangements to install <br />the restroom. Provisional permits may not Ue rene�oed more than once for <br />the same establishment. <br />The health department is enclosing the permit application and the fee <br />schedule. Please complete the permit apolication and return �•iith the <br />permit fee as well as the plan review charge. <br />7. In 1.930, the Stiohomish Healtn District's Eoard of Health establishec'_ <br />a plan review charge for review of all la�out plans and specifications. <br />The fee schedule is $25.00 ner hour to a maximum of $250.00. Your <br />total review time is two hours. Therefure, there �oill be a$50.00 <br />charge. The fee must be paid nrior to issuing your provisional permiC. <br />As I mentioned to you that if you would have presented your layout plans and <br />specifications to both the Snohomish Health DisCrict and to the City of <br />Everett Building Department prior to consCruction, it �aould have done the <br />follo�aino : <br />1.
