Project: 41St St to WMVD Freight Corridor Improvements Change Order No.1 Change Order Effective Date: December 19, 2016 Bid Item # 72—Uniformed Police Officer. Add new force account bid item# 72 entitled, "Uniformed Police Officer" for intersection control work, by adding $75,000.00 to this force account bid item, resulting in a$75,000.00 increase to the Contract price. Bid Item# 73—Class 4,000 Cement Concrete Sidewalk, 6-Inches Thick. Add new bid item# 73 entitled, "Class 4,000 Cement Concrete Sidewalk, 6-Inches Thick" at the agreed to unit price of $54.83/SY, by adding 75 square yards of Class 4,000 Cement Concrete Sidewalk, 6-Inches Thick to the Contract, resulting in a$4,112.25 increase to the Contract price. Bid Item # 74—Type E2 Cement Concrete Curb,3-Day Mix. Add new bid item# 74 entitled, "Type E2 Cement Concrete Curb, 3-Day Mix" at the agreed to unit price of$31.64/LF, by adding 115 linear feet of E2 Cement Concrete Curb, 3-Day Mix to the Contract, resulting in a$3,638.60 increase to the Contract price. Bid Item# 75—Class 4,000 Cement Concrete Curb & Gutter, Type A-1. Add new bid item# 75 entitled, "Class 4,000 Cement Concrete Curb & Gutter, Type A-1" at the agreed to unit price of $31.85/LF, by adding 200 linear feet of Class 4,000 Cement concrete Curb & Gutter to the Contract, resulting in a$6,370.00 increase to the Contract price. Bid Item # 76—42-Inch Black Polyvinyl Chain Link Fence. Add new bid item# 76 entitled, "42- Inch Black Polyvinyl Chain Link Fence" at the agreed to unit price of$48.75/LF, by adding 90 linear feet of 42-Inch Black Polyvinyl Chain Link Fence to the Contract, resulting in a$4,387.50 increase to the Contract price. JUSTIFICATION Bid Item #61 —Force Account Lean Concrete. This bid item is being deleted because lean concrete will not be used on this project. Bid Item # 68—Force Account Controlled Density Fill. This new bid item is being added to account for costs needed to pay for controlled density fill; dry or wet CDF may be used. This change order also adds the CDF Specification in the City of Everett Design and Construction Standards, section 3-20.1 to use for materials acceptance. Bid Item # 69—GarretCom Ethernet Switch. The Contract required the Contractor to provide the GarretCom Switches but did not provide a method of payment to the Contractor. This new bid item is being added to pay for Contractor provided GarretCom Ethernet Switches for this project. Bid Item # 70—CCTV Camera Replacement Cost Difference. The CCTV cameras listed in the Contract are no longer available and are being replaced with a camera that is available and is closest to the original model.
