Final approval is subject ro applicahle codes and field inspections. These <br />documents shall remain on file and available, in good condition, for inspection at the job <br />site until the project is completed. I recommend these documents become a permanent part <br />of the facility records. <br />The Department of Health will not authorize inspections for occupancy of this facility until <br />all phases have been submitted and approved. New facilities should apply for an <br />appropriate license through Facilities and Services Licensing at least six weeks prior to the <br />anticipated opening date. <br />I have enclosed a notification card.
