EXHIBIT A 4 <br /> SCOPE OF WORK <br /> CITY OF EVERETT <br /> ENGINEERING SERVICES <br /> FOR <br /> RESERVOIR NO. 3 RE-LINING PROJECT <br /> The Scope of Work to be completed by MWH Americas, Inc (the Consultant) for the Reservoir No. 3 <br /> Re-Lining Project (the Project) for the City of Everett (the City), Washington is divided into three <br /> phases as described below: <br /> Phase I—Design Engineering <br /> Phase II—Construction Services <br /> Phase III—Undefined Services <br /> The following Scope of Work has been developed for Phase I and Phase II only. This Scope of Work <br /> was assembled with specific input from City staff for completing construction of Reservoir'No. 3 Re- <br /> Lining improvement by December 2007. The Scope of Work for Phase I covers design tasks. Phase II <br /> covers field engineering type services including construction quality assurance such as field inspection <br /> and testing. No post-construction support services have been defined in this agreement. As it is difficult <br /> to accurately determine the nature of all features required for this project a contingency fund of $20,000 <br /> is included in Phase III. The funds included in Phase III cannot be utilized without prior written <br /> approval from the City of Everett. <br /> PROJECT OVERVIEW <br /> The Reservoir No. 3 Re-Lining Project consists of the following elements: <br /> • installation of a new liner on the floor of the City of Everett's existing Reservoir No. 3, <br /> • replacement with new liner material on the sloping walls of the reservoir, and <br /> • construction of a splash pad or other energy dissipation structure beneath the inlet pipe. <br /> It is assumed that no structural repairs to the reservoir will be required. Due to restrictions in the City's <br /> operations, construction must be completed within a time frame beginning approximately November 15, <br /> 2007 and ending approximately December 15, 2007. <br /> PHASE I —DESIGN ENGINEERING <br /> 1.1 Project Management. Manage the activities of in-house and subconsultant work including setup of <br /> files, subconsultant agreement, monitoring budgets, schedule, personnel assignments, monitoring <br /> quality assurance/quality control (QA/QC) activities, and processing of invoices/monthly status <br /> reports. <br /> 1.2 Project Kick-off Meeting and Site Visit. Schedule a project kick-off meeting and site visit within <br /> two days of the City's notice to proceed. This meeting will be held with City staff, and two MWH <br /> staff members.This task will include the following activities: <br /> • Review existing project documentation such as design and construction records, shop drawings, <br /> inspection reports and other information on the existing Reservoir No. 3 facility. <br /> 0� <br />
