2 Project: Sewer System Replacement "F" Sch C Change Order No. 1 Change Order Effective Date: August 14, 2007 Item #45—Crushed Rock for Conduit Crossings, Kromer. Create a new bid item entitled "Crushed Rock for Conduit Crossings, Kromer" at the unit price of$43.00 per ton, for an estimated 195 tons, for an estimated contract price increase of$8,385.00. Exhibit B—Justification: The existing concrete surface and underlying voids in the 3300 and 3400 block of Kromer has deteriorated to the point that patching was impractical and unsafe. New Bid Items Item #37 —Concrete Removal and Subgrade Prep. This bid item was created so that the existing roadway and underlying materials could be removed and replaced. Item #38— Crushed Surfacing Top Course Kromer This bid item was necessary to create due to the contract change to place, grade and compact the material in place. Item #39— Cement Concrete Pavement, 6 inch (3Day). This bid item was created to set forms, place and finish the new concrete roadway. Item #40— Concrete Integral Curb w Rebar Hoop Tie. This bid item was created so that new curb can be installed and finished over the new concrete roadway. Item #41 — Concrete Sidewalk, 4 inch (2'x2' tooled pattern). This bid item was created so that historical concrete sidewalk could be installed and finished. Item #42 — Concrete Driveway, 6 inch. This bid item was created so that new concrete driveways could be installed and finished. Item #43 — Concrete Curb Ramp w/Truncated Domes. This bid item was created so that new concrete wheelchair ramps could be installed and finished. Item #44 — Conduit Crossings on Kromer. This bid item was created so that in the future, existing overhead utilities may be lowered and installed without excavating into the new concrete roadway pavement. Item #45— Crushed Rock for Conduit Crossings, Kromer.
