2007 STOP Violence Against Women Formula Grant <br /> `7 Intergovernmental Agreement <br /> For victim service agencies, each agency shall submit their data electronically as follows: <br /> The CONTRACTOR shall submit data quarterly in InfoNet, relative to the provision of <br /> STOP Grant services. Report data will be due in InfoNet no later than January 15, 2008; <br /> April 15, 2008; July 15, 2008; and October 15, 2008. <br /> The CONTRACTOR shall submit non-personally identifying demographic, service and <br /> compliance data required by state and federal funding sources in the InfoNet data <br /> collection system. The CONTRACTOR shall maintain documentation and records that <br /> support the data reported in InfoNet. <br /> Non-personally identifying client demographic, service and compliance data shall be <br /> entered into, and updated in, InfoNet at least quarterly beginning with clients on whom an <br /> intake is generated on October 1, 2007 and thereafter. The first quarter data (October 1, <br /> 2007—December 31, 2007) shall be collected on all new client intakes as of October 1, <br /> 2007 and entered into InfoNet as soon as possible after the end of the first quarter,but no <br /> later than January 15, 2008. <br /> The CONTRACTOR shall establish and maintain written procedures for the security of <br /> InfoNet use at its site. Procedures shall include: <br /> • Only authorized staff are provided access to the InfoNet data and files; <br /> • Staff are informed of the need for security and confidentiality of data and files maintained <br /> in or available through the InfoNet system; and <br /> • That the CONTRACTOR shall notify the DEPARTMENT Program Coordinator for this <br /> contract when an employee is no longer authorized to access the InfoNet system. <br /> If your agency was provided a computer by the DEPARTMENT for the InfoNet system, <br /> you must notify the DEPARTMENT Program Coordinator in writing prior to the disposal <br /> of the computer or transfer of the computer for another use. The DEPARTMENT <br /> reserves the right to claim the computer for the DEPARTMENT'S use. <br /> For criminal justice agencies, law enforcement and prosecution shall submit their data on a <br /> report form as follows: <br /> The CONTRACTOR shall submit two(2)progress reports on a form provided by the <br /> Department for the work performed during the previous reporting period. Reports are due to <br /> the DEPARTMENT January 15,2008 (for the period October 1, 2007 through December <br /> 31, 2007), and October 15,2008 (for the period January 1, 2008 through September 30, <br /> 2008). <br /> 9. MAINTENANCE OF RECORDS <br /> The parties to this Agreement shall maintain books, records, documents, other evidence, <br /> and accounting procedures and practices that sufficiently and properly reflect all direct <br /> and indirect costs of any nature expended in the performance of this Agreement.
