Project: Sewer I, Water J Replacement <br />Change Order No.1 <br />Change Order Effective Date: January 15, 2008 <br />Net contract price increase as a result of adding the above twelve new bid items comes to $296,018.67. <br />Net contract price increase from modify seven existing items and adding twelve new bid items to the <br />contract then comes to a $556,483.67. <br />JUSTIFICATION: <br />Justification for the modification of the existing bid items. <br />Bid Item #4 — Traffic Control Labor. Additional hours of Traffic Control Labor are required beyond <br />the quantities allowed for in the original bid proposal. <br />Bid Item #6 — Street Cleaning & Sweeping. Additional hours of Street Cleaning & Sweeping are <br />required beyond the quantities allowed for in the original bid proposal. <br />Bid Item #15 — Concrete Pavement Patch. Additional square yards of Concrete Pavement Patch are <br />required beyond the quantities allowed for in the original bid proposal. <br />Bid Item #16 — Gravel Borrow. Additional tons of Gravel Borrow are required beyond the quantities <br />allowed for in the original bid proposal. <br />Bid Item #26 — Combined Sewer Pipe, 21 -inch diameter, PVC F679 Type 1. The design called for <br />the use of the pipe bursting method to install 173 linear feet of 24 -inch diameter, HDPE, DR 17, Pipe <br />in 15th Street, from Oakes Avenue to the alleyway between Oakes and Lombard Avenues. After <br />installation the grade of the pipe was not adequate and the bursting method heaved the roadway surface <br />thus requiring this 173 linear foot section of sewer pipe to be reconstructed with Combined Sewer <br />Pipe, 21 -inch diameter, PVC F679 Type 1 pipe under bid item #26. Additionally the designed called <br />for installation of 480 linear feet of Combined Sewer Pipe, 24 -inch diameter, HDPE, DR 17, Pipe by <br />the pipe bursting method in the 1400 block of Oakes Avenue. To avoid a similar experience as what <br />occurred with the failed 15th Street Pipe Bursting incident, new combined Sewer Pipe, 21 -inch <br />diameter, PVC F679 Type 1 Pipe was installed in the 1400 block of Oakes Avenue, in -lieu -of the as <br />designed method. The net result was a 653 linear foot increase in use of the pipe under bid item 926 <br />and a 480 linear foot decrease in the use of pipe under bid item #29. <br />Bid Item #29 — Combined Sewer Pipe, 24 -inch diameter, HDPE, DR 17, Installed by Pipe <br />Bursting. See justification underbid item #26. <br />Bid Item #42- Concrete Sidewalk Restoration, 4 -Inch Thick. Additional square yards of Concrete <br />Sidewalk Restoration, 4 -Inch Thick are required beyond the quantities allowed for in the original pipe <br />proposal. <br />Justification for the Addition of the New Bid Items. <br />New Bid Item #49- Concrete Roadway. <br />New Bid Item #50 — 6 -Inch Curb Machine Cast. <br />New Bid Item #51— 6 -Inch Curb Hand Formed. <br />New Bid Item #52 — Concrete Roadway Removal. <br />Page 5 of 6 - <br />
