_I5 <br /> EXHIBIT "A" <br /> •Senior Center Projects <br /> SPECIFIC TERMS AND CONDITIONS <br /> I. DOCUMENTS INCORPORATED BY REFERENCE <br /> In performing the services under this Contract, Everett Senior Center (hereinafter <br /> the Contractor) shall comply with the provisions contained in the following <br /> documents incorporated by reference and maintained on file at the Snohomish <br /> County Human Services Department, Long Term Care & Aging (hereinafter the <br /> County): <br /> A. Snohomish County Long Term Care & Aging's Program Instructions; and <br /> B. Contractor's grant application as submitted. <br /> II. REPORTING REQUIREMENTS <br /> The Contractor shall submit required reports on a format supplied or approved by <br /> Snohomish County Long Term Care & Aging. Snohomish County Long Term <br /> Care & Aging shall submit to the Contractor Program Instruction(s) which specify <br /> required reporting information and instructions for completing reports. Overdue <br /> reports shall delay payment to the Contractor until the next billing month. <br /> Report Titles Due <br /> Progress Report July 10, 2008 <br /> Final Report January 10, 2009 <br /> III. REIMBURSEMENT <br /> A. The Contractor must submit requests for reimbursement no later than ten <br /> (10) days following the end of each calendar month. The request for <br /> reimbursement must be submitted on forms approved by Snohomish County <br /> Long Term Care & Aging. The County shall process correct and properly <br /> submitted billings for reimbursement after all supporting documentation is <br /> received. Billings received after the 10th of the month will be processed for <br /> reimbursement in the succeeding month. <br /> B. The monthly billing shall be based on program expenses and be <br /> accompanied by monthly expenditure reports showing line-item expenditures <br /> corresponding to the Approved Contract Budget, or Amended Contract <br /> Budget, Exhibit C. <br /> 115 <br /> Exhibit A <br /> A-08-75-04-198 <br /> City of Everett <br /> Page 1 of 2 <br />
