6 <br /> 3. Survey the pothole locations that were performed to investigate utility conflicts (if <br /> any) for inclusion on the base maps. This will require a separate mobilization <br /> than the design survey, as described above,but is included in the attached Perteet <br /> scope of work. This scope assumes that surveying will be required at up to 10 <br /> potential utility conflict potholes. <br /> 4. Establish reference points for right-of-way and survey control monuments in the <br /> project area. Should any monuments be disturbed by the construction contractor, <br /> the construction contractor shall replace the monument including case and cover <br /> and Perteet will re-mark the point and document the final survey monument data. <br /> Perteet will obtain any necessary permits including the Department of Natural <br /> Resources (DNR) "Application for Permit to Remove or Destroy a Survey <br /> Monument" for up to three(3) monuments that are anticipated to be removed or <br /> destroyed during construction. Perteet will also complete and submit DNR's <br /> "Completion Report for Monument Removal or Destruction" form once the <br /> monument has been reestablished. <br /> Assumptions: <br /> 1. It is estimated that there will be three(3) monuments disturbed by the <br /> construction contractor. <br /> Deliverables: <br /> 1. Provide paper copies and electronic copies of base mapping at scale of 1"=20' <br /> and topographic contours at 2' intervals per City CAD and survey standards. <br /> Base mapping will be performed from right of way line to right of way line. That <br /> portion of the base map outside the area of the design project shall also meet all <br /> requirements of the City's CAD and Survey Standards. <br /> 2. Provide copies of field notes, City field book with survey control data, computer <br /> listings and computer readable files of the survey data points organized into a 3- <br /> ring binder per City direction. The Survey Control binder shall include the survey <br /> control and existing sewer location/elevation data that was obtained for the pre- <br /> design effort. Survey information collected outside the area of the design project <br /> shall also meet all requirements of the City's CAD and Survey Standards. A draft <br /> version of the binders shall be submitted at the 60% design stage. <br /> Task 3—Sewer Replacement Design <br /> This task involves data gathering, preliminary design, preparation of plans, details, <br /> specifications and contract documents suitable for public works permitting, bidding <br /> and construction. Plans and specifications will be presented at the 60%and 90% <br /> complete level for complete technical review by the City.
