City, <br /> AMENDMENT NO. 1 - TO PROFESSIONAL SERVICES AGREEMENT BETWEEN <br /> THE CITY OF EVERETT <br /> AND <br /> PanGEO <br /> WHEREAS, PanGEO and the City entered into an Agreement dated June 5, 2007 ("Agreement") <br /> to provide architectural and engineering services for the Everett Animal Shelter; and <br /> WHEREAS, the total amount of compensation stated in the Agreement is insufficient for the <br /> remainder of the project; and <br /> WHEREAS, an additional $15,000.00 of contract value is needed to complete the project; and <br /> WHEREAS, the City desires continued assistance from PanGEO for geotechnical engineering <br /> services on this project; and <br /> WHEREAS, the City and PanGEO agree to amend their Agreement to increase the maximum <br /> compensation by an additional $15,000.00 from the current level of$18,808.00 to $33,808.00; <br /> NOW THEREFORE, the City of Everett and PanGEO agree to amend and modify the previously <br /> amended Agreement as follows: <br /> 1. The following work is added to the Scope of Work described in Exhibit A of the <br /> Agreement: <br /> Geotechnical Construction Support Services: Generally, construction support services <br /> will be performed on an on-call basis as requested by the contractor or on a periodic basis <br /> to confirm the adequacy of the earthwork construction. After each site visit, PanGEO <br /> will prepare a daily field memo of their observations and any recommendations. The <br /> memos will be sent to the City, applicable members of the design team, and the site <br /> contractor. The following discusses specific areas of PanGEO review. <br /> a. Preconstruction Meeting: PanGEO will attend a preconstruction Meeting with the <br /> City and the construction team to discuss reporting requirements and areas requiring <br /> special geotechnical inspection. <br /> b. Erosion Control Reviews: PanGEO will perform periodic site visits to confirm that <br /> the site contractor is following best management practices to mitigate off site <br /> sediment transport. <br /> c. Building Pad Fill: PanGEO will provide a representative on-site on a near full time <br /> basis for the placement of building pad fill. PanGEO's extended presence is required <br /> for this activity because the dikes surrounding the site, which will be the source of the <br /> material for the building pad,have been found to contain woody debris and organics, <br /> 3 v.. which are unsuitable for use beneath the building pad.
