Exhibit"A"—Scope of Services <br /> City of Everett <br /> 41st Street/Broadway Arterial Improvements <br /> Phase III: Construction Observation & Administration Services <br /> Supplemental Agreement No. 5 <br /> INTRODUCTION <br /> This project involves the arterial improvements to 4151 Street/Broadway project. <br /> Final plans and contract documents have been completed for the improvements. A preliminary <br /> construction critical path schedule was prepared during the design phase indicating that the <br /> project will require a 140 working day construction schedule. <br /> During this phase of work,Perteet Inc. (Perteet) will provide construction observation and will <br /> assist the City of Everett(City)with construction administration services as described in the <br /> following pages as requested by the City. <br /> The Consultant will endeavor to protect the City of Everett against defects and deficiencies in the <br /> work of the Contractor, but he cannot guarantee the Contractor's performance and he shall not be <br /> responsible for construction means, methods, techniques, sequences, procedures for safety <br /> precautions and programs in connection with the work. <br /> GENERAL SCOPE OF SERVICES <br /> This agreement includes professional services to provide alternative analysis, community <br /> involvement and preliminary design for the 41St Street/Broadway Arterial Improvements project. <br /> To allow flexibility,the services under this agreement were completed as follow: <br /> Phase I—Alternative Analysis &Preliminary Engineering Services <br /> Phase II—PS&E Design Services <br /> The final phase of the project was described within the original agreement as follows: <br /> Phase III - Construction Observation& Administration Services <br /> The overall objective of this document is to supplement the 41st Street/Broadway Arterial <br /> Improvements project to provide construction observation and administration services. <br /> Provided below is a description of services to be provided by Perteet, Inc. and subconsultants for <br /> Phase III. <br /> Federal funds will be used in the planning, design, and/or construction of the project. <br /> Accordingly,the procedures outlined in the Local Agency Guidelines (LAG manual) will be <br /> used during this project.
