. _< 3. N 9 CH2M■ ■ILL Exhibit A—Scope of Work Amendment No.4 CITY OF EVERETT 41ST STREET ARTERIAL DRAINAGE DESIGN, CONSTRUCTION PHASE FOR PERTEET,INC. April 25,2008 • Purpose This scope of services is Amendment 4 to the original scope of services between Perteet and CH2M HILL(SUBCONSULTANT),contracted August 16,2006.The scope of work for Phase II(Final Design)under Amendment 1 has been completed to date. This scope of services includes project management and construction design services as described below. Task 5 — Construction-Phase Services Task 5.1 Field Support Subconsultant will provide field support for the City of Everett during construction as required to address construction issues related to stormwater drainage facilities. Task 5.1 Assumptions: • Perteet and/or the City will provide a construction inspector who is primarily responsible for daily inspection and management of construction activities,including the tracking and review of material quantities,submittals,change orders,pay estimates, record drawings,etc. • While providing construction observation assistance,the Subconsultant shall assume no responsibility for proper construction techniques and job site safety.The Subconsultant cannot guarantee the Contractor's performance and shall not be responsible for construction means,methods,techniques,sequences of procedures,or for safety precautions and programs in connection with the work performed by the construction contractor(s),and any subcontractors.
