The conditional approval of the plans for The 12`" Cup Inc was based upon the plans submitted <br /> June 24, 2014 and the menu submitted June 24, 2014. Any changes to the plans suhmitted June <br /> 24, 2014 and the menu submitted June 24, 2014 will void this approval. <br /> 2. The Health District operating permit application process must be compieted prior to opening for <br /> business. This facility vdill be classified 2s a low risk food stand concession �.vdh 0•12 seats. <br /> 3. The approval is for the stand and the submitted commissary oniy. <br /> 4. Hot water must be available to all handv✓ash sinks within 15 seconds. The hot water fpr the facility must <br /> not be used, at any plumbing fixture, for a minimum of 6 hours prior to the preoperational inspection. <br /> This is required so that proper testing of the 15-second time requirement for hot water availability can be <br /> completed. <br /> 5. All food service equipment, both new and used, must be listed by the Nationa!Sanitation Foundation <br /> (NSF) or equival2nt for its intended use. Used and new refrigeration must be capable of holding food at <br /> temperature of 41°F or below <br /> 6. Extra vdall protection is required on walls behind all hand wash sinks. Sinks in corners must also have <br /> r�all protectien on the side walls. A 16-inch high backsplash of plasUc laminat s, fiberglass-reinforced <br /> plastic or equal is acceptable. Wall protection behind mop sinks must cover the entire splash zone. <br /> Plastic coated hardboard is not acceptable. <br /> 7. All floors in the kitchen, food preparation, food service, food storage, food display, and dishwashing <br /> areaS must be surfaced v�ith a durable, nonabsorbent, easily cleanable material. Expansion joints, <br /> seams, saw cuts and the like in concrete floors in all areas, induding customer seating areas, must be <br /> fillad and sealed so as to provide a smooth and cleanable surface. <br /> 8. The ceiling above ihe food storage and display areas must be nonabsorbent, smoolh and easily <br /> cleanable. <br /> 9. Cabinet shelving must be nonabsorbent, smooth and easily cleenable. (Self slick vinyl or other type <br /> surfaces are not acceptable) <br /> i.; . <br /> 10.
