SECTION 01575 <br />HAZARDOUS AND CONTAMINATED SUBSTANCE HEALTH AND SAFETY <br />A. Personal Protective Equipment and monitoring equipment to conform to requirements set <br />forth by Federal and Stale regulations. <br />PART 3 - EXECUTION <br />3.01 PREPARATION <br />A. Prepare and implement a HCSHSP in accordance with the requirements of WAC 296-843. <br />Submit the plan to the Resident Engineer for review in accordance with Section 01330, <br />Submittals. Include, at a minimum, the following site specific information: <br />1. Site Description and Evaluation <br />2. Names of key personnel and alternates responsible for site safety and health <br />(responsible party and chain of command) <br />3. Site specific safety and health hazard assessment and risk analysis based on <br />contaminants known or expected to be present. <br />4. Training <br />5. "Personnel Protective Equipment <br />6. Medical Surveillance <br />7. Air Monitoring <br />8. Site Control Measures (Work Zones, Communications and Security) <br />9. Personnel Hygiene and Decontamination <br />10. Equipment Decontamination <br />11. Logs, Reports, and Record Keeping <br />12. Heat and/or Cold Stress Monitoring <br />13. Emergency Response including excavation routes and procedures <br />14. Site Specific Hazard Communication <br />15. Material Safely Data Sheets (MSDS) <br />16. Accident Prevention Plan <br />B. Distribute the HCSHSP to contract employees. Require employees to read the plan, sign a <br />compliance statement, and abide by its provisions.
