PROPOSED MITIGATED <br />DETERMINATION OF NON -SIGNIFICANCE <br />SEPA #72-96 <br />V 2g I I <br />DESCRIPTION OF PROPOSAL: The applicant is requesting a Binding Site Plan of two <br />(2) lots and Building and Public Works permits to construct a 2,813 square foot AM/PM <br />Mini Mart on proposed Lot A. <br />PROPONENT: ARCO <br />19516 222nd Avenue SE <br />Woodinville, WA 98072 <br />LOCATION: 9905 19th Avenue SE (Tax # 172805-3-032-0000) <br />LEAD AGENCY: Everett Planning/Community Development <br />Department <br />CONTACT PERSON: Don Wood PHONE: 259-8731 <br />The environmental impacts of this proposal are documented in the Environmental <br />Checklist and other information on file with the City. The listed requirements are placed <br />in response to our review of this information: <br />An application and plan review for approval of your Mini Mart is required by the <br />Snohomish Health District. Your contact person is Robert A Hoppa, R.S. at <br />(206) 339-5250. <br />Any grading/fill on this site shall be done so as to not impact the surrounding <br />properties. (SEPA Earth, Land and Shoreline Use Policies) <br />2. Prior to final Binding Site Plan Approval or issuance of Public Works Permits and <br />Building Permits, the Applicant must pay the required traffic mitigation fee for the <br />ARCO Mini Mart of $17,700.00 per the Interim Traffic Mitigation Ordinance. <br />(SEPA Transportation Policy) <br />3.
