TIMING LIMITATIONS: The project may begin immediately and shall be completed by October <br />15, 2006. <br />2. NC�TIFICATION REQUIREMENT: The Area Habitat Biologist listed below shall be contacted by <br />phcne or e-mail (halsegh@dfw.wa.gov) within seven days of completion of work to anange for a <br />compliance inspe�tion. The notification shall include the permittee's name and the control number <br />for this' �PA. <br />3. Work shall be accomplished per plans and specifications approved by the Washington <br />Department of Fish and Wildlife entitlad 23 Unit Townhomes, 7th Ave, Everett Washington dated <br />"printed" Nov�mber 15, 2005 and verbal modification dated December 7, 2005, except as modified <br />by this HydrQ:�lic Project Approval. A copy of these plans shail be available on site during <br />construction. <br />CULVERT PROVISIONS <br />4. The culvert shalt be installed in the dry or isolation from the stream flow by the installation of a <br />bypass flume or culvert, or by pumping the stream flow around ihe work area. Care shall be taken <br />so that the stream below the project area is never dewatered. At least half the flow of the stream <br />shall be maintained in the downstream reach at all times. <br />5. The culvert shall be placed on a gradient no greater than 1% with the bottom of tfie culvert <br />placed below the level of the streambed a minimum of 20 percent of the culveR diameter for a <br />round culvert. The 20 percent placement below the streambed shall be measured at the culvert <br />outlet. <br />6. The cuivert shall be installed to maintain struclural integrity to the 100-year peak ilow with <br />consideration of the debris likely to be encountered. <br />Page 1 o( G <br />
