Willow trees shall be planted among the rocks of the outfall energy dissipater and flow spreader <br />at a density of at least 3 feet on center, and maintenance shall be adequate to ensurE normal <br />growth and survival of at least 80% after 3 years. <br />20. The dissipater shail be large enough so that ihe conveyance o( stormwater will continue to be <br />adequate even when the willows are full size and density. <br />CONDUIT PROVISIONS <br />21. Conduit alignment shall be as nearly perpendicular to the stream as possible. <br />22. The conduit shall be installed at su�cient depth so that subsequent disturbance of the <br />streambed is avoided. <br />23. When using trench excavation, the trenches shall be excavated in the dry or shatl be isolated <br />from the Flowing stream. Disturbance of the streambed shall be limited to the amount necessary to <br />complete the project. <br />24. The trench shall be backfilled with existing streambed material or clean, rounded, stream "fish <br />mix" gravel. The streambed shall be returned to preproject condition. <br />25. Excess spoils shall be disposed of so it will not re-enter the slream. <br />26. The conduit approach trench shall be isoiated from the stream until laying of the conduit across <br />the stream occurs. <br />GENER.AL PROVISIONS <br />27. All bare earth areas shall te protected from erosion and appropriately revegetated prior to final <br />inspection or completion of the project. <br />28. Equipment used for this projecc shall be free of external petroleum-based products while <br />working around the stream. Equipment shall be checked daily for leaks and any necessary repairs <br />shall be completed prior to commencing work activities along the stream. <br />29. Equipment cr�ssings o( the stream are not authorized by this HPA. <br />30. If at any time, as a resull o( project activities, fish are observed in distress, a fish kill occurs, or <br />water quality problems develop (including equipment leaks or spilis), immediate notification shall be <br />made to the Washington Department of Ecology at 1-800-258-5990, and to the Area Habitat <br />Biologist lisled below. <br />Page 3 of 6 <br />
