This Agreement may be amended only upon written agreement <br /> of the parties, executed in the same manner as provided by law for the <br /> execution of this Agreement. <br /> 6. Duties of the Contractor. <br /> 6.1 Services. During the period of this contract, the Contractor will <br /> partially fund the following positions up to the grant amount of$15,588.00: <br /> (a) An advocate specialist and intervention specialist to provide community- <br /> based advocacy services for victims of sexual assault, age 18 and older. <br /> Services provided can include, but are not limited to, information and <br /> referral, crisis intervention, medical advocacy, legal advocacy, intakes and <br /> assessments prior to entry into therapy services, assistance accessing <br /> Crime Victims Compensation benefits, and the development of individual <br /> safety and treatment plans.. <br /> (b)An advocate coordinator to provide staff support and oversight for the <br /> contract. <br /> The Contractor shall work in cooperation with the City and the Snohomish <br /> County Prosecuting Attorney's Office in carrying out its responsibilities <br /> under STOP GRANT No. 2008-WF-AX-0008. <br /> Services provided by the advocates shall meet the requirements set forth by <br /> Washington State RCW 70.125, and by the standards defined by the State <br /> of Washington Sexual Abuse/Assault Services Standards. <br /> 6.2 Community Response Team. Contractor shall participate in <br /> enhancing the Snohomish County Coordinated Community Response Team <br /> to increase the safety of victims of domestic violence, sexual assault, or <br /> stalking and hold offenders accountable. The Snohomish County <br /> Coordinated Community Response Team shall meet monthly, or at least <br /> quarterly, during the contract period with a focus on issues related to <br /> responding to adult victims of stalking, domestic violence, and/or sexual <br /> assault, including victims from marginalized communities. <br /> 6.3 Reporting. The Contractor shall submit data quarterly in InfoNet, <br /> relative to the provision of services. Report data is due in InfoNet no later <br /> than January 15, 2009; April 15, 2009; July 15, 2009 and October 15, <br /> 2009. Contractor shall submit non-personally identifying demographic, <br /> service and compliance data required by state and federal funding sources <br /> in the InfoNet data collection system. The Contractor shall maintain <br /> documentation and records that support the data reported in InfoNet. <br /> Non-personally identifying client demographic, service and compliance data <br /> shall be entered into, and updated in, InfoNet at least quarterly beginning <br /> with clients on whom an intake is generated on October 1, 2008 and <br /> thereafter. First quarter data (October 1, 2008 — December 31, 2008) shall <br /> be collected on all new client intakes as of October 1, 2008 and entered into <br /> InfoNet as soon as possible after the end of the first quarter, but no later <br /> than-January 15, 2009. <br /> After the end of the first quarter, for clients served during the period October <br /> 1, 2008 — September 30, 2009, all non-personally identifying demographic, <br /> service and compliance data shall be entered into InfoNet no later than <br /> October 15, 2009. rs <br /> AGREEMENT RE STOP GRANT Page 2 <br />
